"I'm worried about my parent's safety."
Sometimes the people closest to an older adult do not realize how much their abilities have changed. I begin with a comprehensive assessment of needs, abilities, living situation, current services, and support system. I also listen to the older adult and family members to understand what matters most to each of them.
"My parent needs more help, and I can't take anything else on."
Linden can take on the practical work of coordinating care. We will talk at the beginning about what you want me to handle and what you want to remain involved in. That level of support can change as your parent's needs change.
"I need someone to keep an eye on things."
Regular check-ins and home visits allow me to get to know your parent and notice changes that might otherwise be missed. I can observe how they are managing, talk with caregivers, look for safety concerns, and make sure the care plan continues to fit.
"My parent is coming home from the hospital."
Returning home after a hospitalization or rehabilitation stay can be overwhelming. I can participate in discharge planning and help arrange the practical pieces needed for a safe transition home.
"We're not sure where my parent should live."
I can help you assess your parent's current living situation, understand the options available, and explore alternatives when a change is needed. The goal is to find an arrangement that is safe, appropriate, and consistent with their wishes.
"I want someone who knows my parent and can help when things change."
Linden provides ongoing care management with regular communication, home visits, care coordination, and updates to the care plan as needed. The level of involvement is different for every family.
